403(b) Plan Participants

Thank you for reaching out to Blossom Philadelphia regarding your 403(b) retirement plan. The Plan was terminated on June 30, 2019 and all balances, with some exceptions, were distributed or rolled over to new accounts according to directions of plan participants. If you did not file paperwork directing your balance to be distributed or rolled over, the balance was moved into an Individual Retirement Rollover Account under the custodial oversight of the Third Party Administrator (TPA), One-America.

If you wish to retrieve or move your balance or have other questions, please relay your request to info@BlossomPhila.org. We will work with you and the TPA to resolve your questions.

Pension Plan Participants

Thank you for reaching out to Blossom Philadelphia regarding your Pension. To assist retirement plan participants, please refer to the following FAQ’s:

Q1 - I recently submitted my lump sum distribution or rollover request – when will my transaction be processed?

Given the Covid-19 impact on daily living, we are pushing back the anticipated date when all distributions or rollovers are made to June 15th, 2020 for participants that have provided us their termination paperwork. Please reach out to us at Info@BlossomPhila.org should you require termination paperwork or have any other questions.

Q2 – Can I find out how much will be distributed to me or rolled over?

We cannot give an exact number as final calculations are in process. If you wish to know approximately how much will be distributed or rolled over, we can provide you with an estimate. Please contact us via email at info@BlossomPhila.org to request an estimate.

Q3 – How can I reach someone at Blossom Philadelphia?

Blossom Philadelphia closed on December 31st, 2018 and the building was sold in May 2019 so please do not mail communications to the former address. Beane Associates, Inc. has been retained by the Receiver in charge of winding down Blossom Philadelphia to assist with the distribution of assets in the retirement plans. Q4 – I need assistance filling out my forms, who should I ask?

Please contact us via email at info@BlossomPhila.org and we will set up a time to call you directly to address your questions.

Q5 – I’ve lost my forms – where can I get new forms?

Please contact us via email at info@BlossomPhila.org and we will send new forms to you. Completed forms should be mailed to Blossom Philadelphia, c/o Beane Associates, 22 The Commons, 3518 Silverside Road, Wilmington, DE 19810-4907.

Tax Reporting

W-2’s – All 2018 W-2’s will be mailed out by Blossom’s subcontractor, Proxus, prior to January 31, 2019. If you need a copy your W-2, please contact us at info@BlossomPhila.org.

All 2019 W-2’s will be mailed out by Blossom’s subcontractor, Proxus, late in 2019 or early in 2020. If you change your address during 2019 please provide your new mailing address to info@BlossomPhila.org so that Proxus can be alerted.

403(b) and Pension Distributions and Rollovers – as you may be aware, Blossom Philadelphia has tax reporting responsibilities associated with 2019 distributions, both for the 403(b) and the Pension Plan, which are reported on Form 1099-R, Distributions from Pensions, Annuities, Retirement or Profit-Sharing Plans, IRA’s, etc. Blossom’s deadline for filing forms 1099-R is February 28, 2020. If you do not receive your 1099-R by February 9, 2020, please email us at info@BlossomPhila.org.

If your distribution or rollover was completed in 2019, you will receive a 2019 1099-R later in the year or in early 2020.
