403(b) Plan Participants
Thank you for reaching out to Blossom Philadelphia regarding your 403(b) retirement plan. The majority of distributions and rollovers were completed in 2018. We are diligently working with our Third Party Administrator (TPA) One-America to manage the remaining participant requests.
If you need forms or have questions about your rollover or distribution, please reach out to us at info@BlossomPhila.org and we will work with the TPA to resolve your problem. If you have not yet submitted your paperwork to enable your distribution or rollover, please mail your completed forms to Blossom Philadelphia, 100 E Mermaid Lane, Philadelphia, PA 19118.
Pension Plan Participants
Thank you for reaching out to us regarding your Pension Plan distribution questions. To assist the retirement plan participants and to support Blossom’s Board of Directors in distributing Plan assets properly, please refer to the following FAQ’s:
- Why have I not received my distribution or when will my rollover be processed?
Your Pension Plan is currently under audit by a CPA firm as required by law. The auditors recommend that no more distributions or rollovers be made until the audit is complete. The audit is expected to be completed by April 15th, 2019.
- Why can’t I get my money now?
It is a required step in the wind-down of a Pension Plan to determine that all calculations are correct. To ensure the next distribution checks will be the final distribution checks, time is needed to confirm all calculations. Rest assured that no one has received a distribution during 2019 ahead of you.
- Can I find out how much will be distributed to me or rolled over?
We cannot give you an exact number as calculations are in process. If you wish to know approximately how much will be distributed or rolled over, we can provide you with an estimate after April 1st.
- How can I reach anyone? When I call Blossom’s phone number, I get an automated response.
Starting March 7th, from 3:00 – 5:00 pm and on every Monday and Thursday until April 11th, from 3:00 – 5:00 pm, Blossom’s Human Resources office will be staffed with people to address your concerns and answer your questions. - I have a question about filling out my forms. Who should I ask?
Please contact us via email at Info@BlossomPhila.org and we will answer your question or set up a time to call you directly to address your question.
- I’ve lost my forms – where can I get new forms?
Please call us directly or contact us via email at Info@BlossomPhila.org and we will send new forms to you. They can be returned to Blossom Philadelphia, 100 E. Mermaid Lane, Philadelphia, PA 19118.
W-2’s – All 2018 W-2’s will be mailed out by Blossom’s subcontractor, Proxus, prior to January 31, 2019. If you have not received your W-2 by January 19th, please alert us at info@BlossomPhila.org.
All 2019 W-2’s will be mailed out by Blossom’s subcontractor, Proxus, late in 2019. If you change your address during 2019 please provide your new mailing address to info@BlossomPhila.org so that Proxus can be alerted.
403(b) and Pension Distributions and Rollovers – as you may be aware, Blossom Philadelphia has tax reporting responsibilities associated with 2018 distributions, both for the 403(b) and the Pension Plan, which are reported on Form 1099-R, Distributions from Pensions, Annuities, Retirement or Profit-Sharing Plans, IRA’s, etc. Blossom’s deadline for filing forms 1099-R is February 28, 2019. If you do not receive your 1099-R by February 9, please email us at info@BlossomPhila.org.
If your distribution or rollover was not completed until 2019, you will not receive a 2018 1099-R but will instead receive a 2019 1099-R later in the year.
